Procedures
- The applicant shall submit the form and the enclosed documents to the Reception Desk officer.
- The form and the enclosed documents will be verified for completeness.
- If the submitted documents are not complete, the applicant will be asked to update the required documents.
- If the submitted documents are complete, the applicant will receive a payment voucher showing the due amount.
- The auditor endorses the voucher.
- The applicant then pays the prescribed fees at the cashier’s counter.
- The new health card is printed and handed over to the applicant.

- A medical report showing the date of pregnancy and the expectation date.
- A copy of the Family Book.
